Comprehensive Chronic Pain Medication Protocol

Long term  narcotic administration in a Chronic ( malignant and  non  malignant) pain patient has become one of the most controversial issues in Pain Medicine. Due to the epidemic proportions of abuse and misuse of prescribed narcotics, the need for a strict and safe  protocol has risen. The protocol includes:

  • Psychological evaluation that assess the risk of addiction and identifies any untreated psychological comorbidities, ( anxiety, depression, sleep disorder)

  • Initial and periodic drug testing (urine or saliva)

  • Narcotic agreement and revision of the Illinois Monitoring Program records.

  • Cognitive impairment screening. This is of critical importance in the geriatric population because of the higher incidence of this problem, which becomes much more apparent yet difficult to manage once these patients are on chronic pain medications.
